I, the cardholder authorized to make charges on the above credit card (the “Card”), hereby authorize Kidswim, Inc. to charge my Card for swimming instruction and related services provided to me, in accordance with Kidswim’s standard billing policies and procedures. Kidswim will bill me on the last day of each calendar month. I agree that if my Card becomes invalid for any reason, I will provide Kidswim with a new, valid credit card for billing purposes. Kidswim will attempt to contact me if an appointment must be cancelled due to rain or instructor unavailability, and I will not be charged for any appointment that Kidswim cancels due to instructor unavailability or weather. Notwithstanding, I agree to be charged for any appointment I cancel on less than twenty-four (24) hours’ notice.
